The ACT Team Registered Nurse functions as an integral part of the Assertive Community Treatment Team to provide intensive, recovery-based treatment for the highest risk consumers of mental health resources.
The ACT Team Registered Nurse manages the medication system; documents medication treatment; communicates and coordinates with medical providers; engages in health promotion, education, and provider training to other team members; and assists consumers with understanding medications prescribed and the importance of taking medication.
*Essential Duties and Responsibilities*
* Work collaboratively as part of the ACT Team in order to provide comprehensive care that meets the individual needs of each client enrolled in ACT;
* Assess client health daily and respond appropriately to any noted concerns (i.
e.
medication side effects, withdrawal symptoms, general health changes, etc.
)
* Monitor client use of medications, changes in medications, and assess for efficacy, safety, and/or side effects of medications;
* Collaborate with all providers involved with client’s health care and provide feedback to providers as needed;
* Link client to appropriate healthcare providers or specialists and follow-up with all referrals and recommendations;
* Provide health and wellness education to the client, the client’s supports, and other ACT Team staff members;
* Participate in the formal training and development process with administrative staff and consultants as well as ongoing professional development and continuing education;
* Attend daily team meetings, individual supervision, and group supervision as scheduled;
* Initiate and maintain relationships with community resources, outside human service agencies, and other stakeholders as needed;
* Commit to ensuring fidelity to the ACT model and ongoing quality improvement efforts;
* Complete all required documentation accurately and on time;
* Other duties as assigned.
*Required Skills*
* Knowledge, understanding, and experience providing mental health and substance use treatment, including experience conducting mental health evaluations, familiarity with psychiatric medications, and awareness of medication assisted treatment (MAT) practices;
* Ability to respond to client needs using a trauma-informed, recovery-oriented approach;
* Comfortable working with clients diagnosed with severe and persistent mental health disorders in a variety of office, home, and community-based settings;
* Ability to function as an integral part of the collective treatment team;
* Knowledge of local, state and federal regulations, laws and standards with regard to behavioral health treatment services;
* Strong oral and written communication skills and ability to communicate effectively with clients, family members, colleagues, and professional contacts;
* Ability to plan and prioritize work and meet the Agency’s performance, quality assurance and productivity standards;
* Commitment to maintain the fidelity and integrity of ACT throughout all phases of development, implementation, and practice.
*Required Education & Experience*
* Registered Nurse, licensed to practice without limitations in Pennsylvania
* Experience working in a behavioral health setting preferred*Travel*
* Local travel within the service delivery area is required as many ACT services are provided in community-based settings;
* Travel to trainings as required.
