*Essential Duties and Responsibilities*
* Assist clients with obtaining information about their benefits and refers clients as needed.
* Assess client’s vocational functioning on ongoing basis utilizing background information and work experience
* Conduct job development and job searching activities directed towards positions that are individualized to the interest and uniqueness of individual
* Provide individualized follow up support services to assist client in maintaining employment
* Provide educational and support to individuals
* Provide timely interventions with returning phone calls and assisting with leads
* Participate in regular meetings with treatment team and communicates individually with team members between meetings to coordinate and organize services into mental health treatment.
* Participate in face to face meetings with rehabilitation counselors to coordinate services for clients.
* Develop an individual rehabilitation plan with clients.
Incorporate input from treatment team members and family member with permission.
* Provided supported education utilizing approved principles.
* Facilitate the completion of assessments for planning and development purposes.
* Assist consumers in locating suitable placements and securing appropriate behavioral health, vocational/educational training.
* Collect and assess relevant, mental, behavioral, psychological, economic, medical, and educational background on consumers.
* Provide supportive counseling to consumers and their families.
* Respond to and assess emergency situations and incidents and assure that appropriate actions are taken to protect the health and welfare of consumers.
* Participate in all aspects of client’s treatment from admission through discharge; serve as a primary source of service coordination for all clients enrolled in ACT;
* Work collaboratively as part of the ACT Team in order to provide comprehensive care that meets the needs of client team members;
* Participate in the development and implementation of a comprehensive, strengths-based, individualized treatment plan in collaboration with staff team members, client team members, and client-identified supports;
* Assist in identifying and accessing community resources, linking clients with those resources, and following up on all referrals and recommendations;
*Required Skills*
* Comfortable working with clients diagnosed with severe and persistent mental health disorders in a variety of office, home, and community-based settings;
* Ability to function as an integral part of the collective treatment team;
* Ability to respond to client needs using a trauma-informed, recovery-oriented approach;
* Knowledge of local, state and federal regulations, laws and standards with regard to behavioral health treatment services;
* Exceptional oral and written communication skills and ability to communicate effectively with clients, family members, colleagues, and professional contacts;
* Ability to plan and prioritize work and meet the Agency’s performance, quality assurance and productivity standards;
* Commitment to maintain the fidelity and integrity of ACT throughout all phases of development, implementation, and practice.
*Required Education & Experience*
* Bachelor’s degree with one year of associated experience
* Master’s degree in education, counseling, rehabilitation preferred
