*Universal Primary Care (UPC)* is seeking an energetic, self-motivated, person to join our nursing team as a full time Tactical Nurse in our Olean, NY clinic! This a newly created position to help support our provider and nursing teams.
This position is primarily Monday - Friday day shift, no weekends and no holidays.
UPC is proud to offer family medicine, Pediatric, and Women's Health care to our patients.
This position is responsible to serve as the hub for all patient care team members seeing patients independently, within nursing scope, and assist the team with complicated cases.
Assist with developing care plan and health maintenance needs for all patients while increasing access in the clinic.
Provides education to patients of complex cases, provides walk-in appointments, patient follow-up, triage and provide advice to patients via phone.
This position may provide direction and guidance to LPN’s, Medical Assistants, office staff and others as appropriate.
This position may provide training and orientation to new staff.
*ESSENTIAL FUNCTIONS*
* Supports and assists Director of Nursing in administration of all immunizations, including but not limited to, VFC (Vaccines For Children) program.
Monitors and reports to New York State, Director of Nursing and to other staff as needed and required.
Responsible for inventory at all sites in cooperation with clinical site coordinators.
* Acts as staff RN, providing leadership, advice and coverage to/for LPN staff.
* Coordinates care of patients with chronic conditions/needs, including, but not limited to, Hepatitis C, MAT, Diabetes and anticoagulant management
* Performs triage duties.
* Demonstrates professional responsibility and accountability in implementing the nursing process.
* Performs procedures in accordance with established company policy and procedures.
* Utilizes equipment according to company polices/manufacturers standards.
* Manages and communicates patient information appropriately.
* Demonstrates the knowledge and skill necessary to provide care appropriate to the age and developmental level of the patient.
* Respects cultural customs and beliefs.
* Collaborates with other members of the health care team in order to optimize patient outcomes.
* Coordinates the use of human and material resources for the provision of care to assigned patients.
* Identifies and provides the patient and family with appropriate information regarding assessed educational needs.
Assumes responsibility for a role in the education of students, staff and community.
* Demonstrates professional growth as evidenced by a minimum of ten (10) hours of related educational in-services annually with five (5) hours pertaining to the primary unit worked.
* Assists in data collection for Quality Assurance Plan.
* Complete pre-visit planning/chart preparation as needed.
* Participates in team huddle.
* Provides patient education as needed.
*EDUCATION/EXPERIENCE*
* Graduate of an accredited school of Practical Nursing
* Current New York State RN licensure required
* Successful completion of BLS course required
* Successful completion of Infection Control training required
* Current Pennsylvania RN licensure preferred
