*Universal Primary Care (UPC)* is seeking an energetic, self-motivated, person to join our nursing team as a full time float LPN! The position will work out of the Bradford, PA office a minimum of two days per week and then provide coverage at our other locations in Olean, Salamanca, Cuba, and Houghton NY as needed.

This position is responsible for performing tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a registered professional nurse or licensed physician or other licensed health care provider legally authorized to do so.
May be asked to assist training and orientation other employees.
This position would travel between all locations providing nursing coverage as needed.
Those locations are Olean, Houghton, Salamanca, and Cuba NY as well as Bradford, PA.
*ESSENTIAL FUNCTIONS*
* Demonstrates accountability for the provision of nursing care
* Assists with provider documents (“folder manager”)
* Collects data and communicates patient information in order to optimize patient outcomes
* Respects cultural customs and beliefs
* Assists in determining the health status and health needs of the patient and assists in the formulation of goals/outcomes
* Demonstrates the knowledge and skill necessary to provide care appropriate to the age and developmental level of the patient
* Assists in the coordination of human and material resources for the provision of care to assigned patients
* Participates with other members of the health care team for patient care delivery
* Demonstrates professional growth as evidenced by a minimum of ten(10) hours of related education annually
* Responsible for working with providers to best efficiently meet their needs and our patients’ needs
* Assists with procedures as appropriate and needed
* Provides stocking, storage, cleaning, communication nurse duties and others as assigned
* Responsible for NYSDOH Rx EMR paper
* Ensures safety of all medicines and medical supplies maintained at the practice site
*EDUCATION/EXPERIENCE*
* Graduate of an accredited school of Practical Nursing
* Current Pennsylvania LPN licensure required
* Current New York LPN licensure preferred
* Successful completion of BLS course required
