*Chronic Care Manager* The Chautauqua Health Network (CHN) is seeking to hire an innovative *Chronic Care Manager *to join our team.
This full-time or part-time position will work in collaboration with the Practice Development Manager on the CHN/CIDS Care Team to develop and implement chronic care management plans as well as support related activities.
The *Chronic Care Manager* will be responsible for providing chronic care management services to primary care patients, supporting bridging care team and accountable referral initiatives and other duties as assigned.
Specifically the Manager will: * Assess patient needs, develop and implement care plans, collaborating with healthcare teams, and advocating for patients to ensure they receive appropriate and cost-effective care.
* Monitor patient progress, provide education, and facilitate communication between patients, families, and healthcare providers to promote positive health outcomes.
* Collaborate with healthcare and social service partners to further accountable referral initiatives.
* Track the progress of referrals to ensure timely and effective care delivery through use of population health tools.
* Support content and identification processes involved in advancing Best Practice and Learning Management System (LMS) efforts.
* Connect with patients enrolled in skilled nursing facilities to provide post acute care navigator support.
* Other duties as assigned by the Management Team.
The Manager will act as the primary contact for chronic care management services and related activities will: * *Utilize *a systematic approach to provide individualized, goal-directed care by developing and implementing comprehensive care plans.
* *Promote *and participate in education and counseling to participants based on health needs including preventative care measures.
* *Communicate *with service delivery participants, partners, physicians and appropriate personnel to provide care coordination.
*The candidate will have strong managerial skills and be able to:* * Effectively multi-task and respond to a variety of situations with attention to detail.
* Possess strong communication in English (and Spanish proficiency preferred but not required), effectively communicating with patients, providers and other stakeholders.
*Licensure, Experience and Skills* * Current professional license in New York State (RN, NP, PA, etc.
).
* Minimum 1-2 years’ experience in a clinical or telehealth setting.
* Demonstrated customer service, communication, interpersonal and problem-solving skills.
