Responsibilities: Reviews and applies dietary orders written by the physician.
Prescribes dietary modifications at the physician’s request to make it easier for the patients to follow their dietary regimen.
Modifications are documented in the patient’s chart.
Develops individualized meal plans for patients to help them stay within their prescribed dietary orders.
Appropriate documentation is noted in patient’s chart.
Interviews patients to obtain diet histories and assess nutritional status.
Provides nutritional advice to physicians upon request to help them better understand what dietary regimen would be most helpful to the patient.
Educated/counsels patients regarding their nutritional needs as it relates to their diagnosis or problem.
Appropriate documentation is noted in patient’s chart Qualifications: Qualifications: Registered Dietitian (R.
D) and Certified Diabetes Educator (C.
D.

) required.

in Nutrition with accredited dietetic internship, preferred.
Two to four years related experience and/or training, preferred.
Equivalent combination of education and experience.
