Looking for engaging, team-oriented, professional LPN with excellent customer service skills in a busy outpatient clinic.
This position will report directly to the Nurse Manager.
Individual should be self -motivated in the pursuit of meeting professional work standards.
LPNs at this grade level are expected to provide direct care and perform all duties within the scope of their license (e.
g.
, medication administration).
Completed work is under the general supervision of a Registered Nurse (RN) .

*Job Description * LPN will work within a Whole Health System of care within the Patient Aligned Care Team.
The Patient Aligned Care Team is patient-centered, advocate for the Veteran in which care is coordinated and timely.
The incumbent be an active member of the PACT team.
The nurse is able to empower and equip people to take charge of their health and well-being and live their lives to the fullest.
The Incumbent will demonstrate proficiency in practice, planning, and self-direction in goal setting for managing each Veteran's medical needs.
Actively supports and actively participates in nursing facility wide initiative, trainings, and/or committees.
Supports and actively participates in initiatives such as High Reliability Organization (HRO), Patient Centered Care, Just Culture, and I CARE.
*Duties include, but aren't limited to: * · Proactively explains procedures to patients, using language which they understand - Knowledgeable, skilled in preparing patients for tests, examinations, and treatments -Prepares and administers medications per license with correct documentation in electronic health record -Recognizes emergent or acute situations, responds appropriately, and reports to the RN or provider promptly · Observes, reports to RN, and documents in the patient’s medical record subjective and objective changes in the patient’s physical or emotional condition, both obvious and subtle, which have relevance in determining the health status of the patient.
*Skills*: - Valid LPN license - Proficiency in using electronic health record system - Excellent customer service & ability to work in an interdisciplinary setting - Competency in wound care, procedures, medication administration - Able to prioritize competing demands while maintaining a positive attitude, self-control and responds consistently to the requirements of the work situation, show flexibility - Demonstrated advocacy for patients, family members and staff - Strong ability to educate patients and family members, as well as staff - Experience as a charge nurse, creating a cohesive supportive team that works collaboratively Must be a U.
S.
Citizen and graduate form approved state agency and/or accredited by NLNAC at time the program was completed.
