RN Home Base Primary Care

RN *Home Based Primary Care (HBPC)* Location;Jamestown, NY Special Qualifications for this position are as follows and must be met by the date of this posting: Previous Geriatric Home Care Experience Two years acute medical/surgical RN experience within the past five years required.
*D*emonstrates sound clinical judgment and assessment skills; medication administration and set up, knowledge of medication interactions and usage, and case management Valid driver's license required.
Able to make sound, independent clinical judgments and accurate physical assessments, ability to organize and prioritize own work, capacity to function effectively as a member of an interdisciplinary team PRI (Patient Review Instrument) certification preferred.
Job Type: Full-time Pay: $60,679.
00 - $100,000.
