*Mental Health Therapist: Mobile Crisis Response * *Job Summary:* Work in conjunction with a peer advocate and other Mobile Crisis team members to provide crisis intervention services in the community.
This position is contracted through Connecting Communities in Action in Cattaraugus County, NY, however, the position is based in Jamestown NY as a partnership with Chautauqua County Department of Mental Hygiene *Education & Experience:* * _*Minimum Educational requirements: Licensed Clinical Social Work (LCSW), Licensed Master Social Worker (LMSW), Licensed Mental Health Clinician (LMHC) or Registered Nurse (RN) with psychiatric experience.
*_ * Experience treating a wide variety of mental health diagnoses.
* Experience in assessing lethality.
* Direct experience with, or significant knowledge of, the intersectionality of mental illness and substance use disorders.
* The ability to effectively deescalate crises.
*Other Requirements & Skills:* * Must have valid driver’s license, insurability, and reliable transportation.
* Must pass all relevant background checks including: NYS Central Register Check, NYS Staff Exclusion Check, Local, State and Federal Criminal Background checks and Department of Justice Finger Printing.
* Ability and motivation to set and accomplish deadlines, work independently, and without direct supervision, as well as in a team and with community partners and members.
* Openness to learning new skills and technology as necessary; must participate in training as required.
*Primary Responsibilities:* * Respond to crisis calls in the community; shall report to crisis setting.
* Assess client needs and lethality.
* Develop a crisis intervention plan, with may include, but not be limited to de-escalation, use of informal supports, respite services, and hospitalization.
* Implement crisis intervention plan with consent of client.
* Provide client and his/her social supports with emotional and tangible support, advocacy and linkage to additional services needed to stabilize immediate crisis.
* Coordinate various activities and treatment programs with other social agencies.
* Collect information and releases of information from client and relevant support systems as needed to coordinate follow up care.
* Compile case files and records pertinent information as required by the funder.
* Complete documentation in a timely manner as outlined in the Chautauqua.

* Maintain and report statistical data necessary for program evaluation.
* Maintain information on community and health resources which can be utilized for client care.
* Participate in the process of transfer of customers to other levels of treatment as appropriate.
* Case conference with other providers as needs to ensure continuity of care.
* Comply with all funding source regulations, for creating a safe environment, and to report any unsafe situations to the immediate supervisor.
* Must be enthusiastic about promoting the mission of CCA, be willing to work towards the good of the entire agency, and to act as an ambassador of CCA throughout the community.
