Job Title: RN Clinical Navigator – Transitional Care
Location: Hybrid - remote and onsite
Travel Required: Varies up to 50%
Position Type: Full-time, Non-Exempt
Pay scale: $23 - $27 per hour
SUMMARY:
The RN Clinical Navigator - Transitional Care role will work with a patient after a hospitalization or other inpatient facility stay.
This role will address the hand-off period between the inpatient and return to primary care assisting the patient or caregiver with a medical crisis, new diagnosis, or change in medication therapy.
ROLE AND RESPONSIBILITIES:
Contact the beneficiary or caregiver within two business days following a discharge.
Coordinate a follow-up visit within 7 days of discharge, depending on the complexity of medical decision making involved.
Medication reconciliation
Obtain and review discharge information.
Review the need for diagnostic tests/treatments and/or follow up on pending diagnostic tests/treatments.
Educate the beneficiary, family member, caregiver, and/or guardian.
Establish or re-establish referrals with providers and services, if necessary.
Assist in scheduling follow-up visits with providers and services, if necessary.
Any additional duties as assigned by management
Requirements:
QUALIFICATIONS AND EDUCATION REQUIREMENTS:
Current CPR Certification obtained within 30 days of hire required
At least two years of healthcare experience in a related setting required
Bilingual preferred
Current Registered Nurse licensure in New York State required

PHYSICAL REQUIREMENTS/WORKING CONDITIONS
The physical requirements and working environment are typical of most medical office settings.
Employees are frequently required to sit for long periods of time, stand, walk, balance, hear, talk, reach, and use their hands.
Employees may occasionally be required to bend, stoop, kneel, crouch, climb (stepstool) and stretch.
Physical effort requires some lifting usually not in excess of 15 pounds.
The office work environment is indoors and free from dust, smoke, moisture, and excessive noise.
