RN Care Manager - Care Management - Full Time Position summary: The Case Manager serves as advocate and facilitator for achieving client wellness and autonomy through advocacy, communication, education, identification, and facilitation of services and resources.
The Case manager acts as the “coordinator of care” facilitating collaboration among the various disciplines to meet the client’s physical, psycho-social and spiritual needs in a cost effective manner.
Qualifications: Current NYS RN license; BSN preferred.
Minimum of five (5) years clinical experience required.
RN-BC through the American Nurses Credentialing Center (within 2 years in the position).
Must demonstrate the expertise in communication and negotiation skills.
Must demonstrate the ability to work with people of varied backgrounds and circumstances in a professional and confidential manner.
Must have an understanding of data analysis and integration.
